You may choose to complete this form prior to bringing your pet in to be boarded with us. This form is not to be used to make a Boarding Reservation. Please call us at 510-521-1700 to make a boarding reservation.

We will feed your pet our stock food if we run out of your food.
Access to toys and chewies will be at the Doctor's discretion. Due to potential choking hazards, raw-hides and similar products will NOT be given to your pet.
In the event that we find fleas on your pet, we will administer one dose of Flea Medication and will add the cost of that medication to your Boarding bill.
